Sage Surfaces must receive the order form to be able to process the product only order.

Two Options for Completing the Order Process 1.Preferred Method: Utilize the Sage Surfaces Portal a) Upload a copy of the order form into the Portal. b) You can attach a saved copy of the fillable order form or a scanned copy of the paper order form. 2.Printed Order Form a) Fill in the form by hand. Scan in the finalized form, save and email to laminate@sagesurfaces.com b) Subject line on email must have Store #, PO #, and Customer’s Last Name example: Store 595_PO# 12345678_Saliby
